If you wish to revoke your Self-Exclusion, please click here. (Please note that this online service requires you to have a printer.) After you have applied to revoke your Self-Exclusion, you will be prompted to download a Referral Letter. Please print the Referral Letter and bring the letter to the National Addictions Management Service (NAMS) for an assessment. After you have completed your assessment and doctor’s appointment (if necessary) and counselling session(s) (if necessary), a report will be sent to NCPG. An NCPG officer will then contact you to make an appointment for you to complete the revocation process. For revocation of Family Exclusion, call the NCPG at 6354-8154 for an appointment. All applicants for revocation of Family Exclusion Order must attend a hearing.
